Care Coordination & Privacy Framework

Love INC of Shelby County works alongside churches, community organizations, and local service providers to help neighbors access practical support such as food assistance, utility support, transportation, counseling referrals, and other stabilization resources.

We are committed to protecting the privacy of every neighbor we serve while coordinating care responsibly with trusted partners.

Our Role
Love INC functions as a community care coordination partner, not a medical provider.
We:
· Receive referrals from churches, community partners and health care providers.
· Connect neighbors to non-clinical support services.
· Coordinate follow-up when appropriate.
· Maintain our own internal documentation systems.

We do not:
· Provide medical treatment.
· Access hospital electronic medical records.
· Maintain hospital clinical records.
· Share medical documentation with third parties.

How We Coordinate With Health Care (HIPAA) Providers
When coordinating with hospitals or other health-related organizations, Love INC communicates only what is necessary to ensure continuity of support.
Information shared may include:
· Client name
· Confirmation of referral connection
· Community resources engaged
· Pending documentation or next steps
We do not share detailed medical diagnoses, test results, medication information, or clinical notes unless necessary for coordination and permitted under applicable privacy standards.

Communication Practices
To safeguard privacy:
· Case coordination meetings are conducted in private with authorized personnel only.
· Phone communication is preferred for case-specific updates.
· Email communication is limited to administrative or minimal coordination information.
· Love INC does not maintain shared digital repositories of external medical records.
· We apply the “minimum necessary” standard in all communications.
